A Guide for Patients and Carers

OUR SERVICE

Our Service

 

 Our service is provided by a team of qualified and experienced specialist nurses, counsellors and care assistants, supported by providers of clerical, financial and administrative services.  To keep indirect costs as low as possible, staff work from their own homes, meeting regularly to discuss individual cases.  The Board of Trustees and the Management Committee (with responsibility for day to day operations) also meet regularly.

 

Patients who are referred to us are assessed by one of our nurses, who will visit the patient and their family, and also contact other healthcare professionals involved, particularly the GP and District Nurse, to assess the type and degree of care required and how this may best be provided.  A care plan is drawn up with the patient and care arranged, with one named carer appointed to take overall responsibility in each case. There is no charge for this care.

 Anyone may refer a patient to us, but normally it will not be possible to take on the care of any patient without the full agreement of their GP.    

As part of our services for patients we are able to offer complementary therapies.

Clinical Review

 

 December through to February is usually a challenge for The Rosemary Foundation Nurses, seasonal colds and ‘flu, the Christmas holiday period, together with our winter weather, can conspire to test us all.  However, it has to be said, in spite of twelve inches of snow and freezing conditions, not to mention the odd viral infection, we coped.  Like the Pony Express, our nurses slid and slithered down country lanes and urban roads, while most traffic remained stationary.  Essential equipment being boots, a shovel and something to put in front of the cars wheels if we became stuck.  Where driving became impossible, our nurses walked to their patients.  No patient was left unattended that needed a visit from us – although our timetable went out of kilter at times.  Sincere thanks to all: to patients and their carers for the concern and understanding,  and to our nurses who braved the wintery weather.

Special mention should be made of the husband of one the our nurses (Jill Boucher, she is pictured in the snow) who drove Jill in his 4-wheel drive vehicle to her patients, waited for her, and then took her on to the next.

In other news, we are delighted to welcome a new member of staff to our team, Mrs Elaine Biles (look out for Elaine’s article) and to say The Foundation received 32 new patient referrals during the months of December, January and February, so we really appreciated the extra pair of nursing hands!

Julie and I have been busy in the office and The Rosemary Foundation ‘Guide for Patient and Carer’ is now available from our website or from the Langrish Office.

HAMPSHIRE PRIMARY CARE TRUST GRANTS

Hampshire Primary Care Trust Grants for Carers

It was with great pleasure that the Rosemary Foundation received a grant to administer, on behalf of Hampshire County Council and Hampshire Primary Care Trust. This grant is to be distributed to carers of people that have an incurable illness and may be used towards a short break, broadband access, sports/ leisure activities or perhaps towards adult education classes.

MAKING LIFE A LITTLE EASIER

Making Life just a Little Easier.

When we are helping you to look after a family member at home never hesitate to ask our nurses about any problems you may have. It may be to do with bathing or showering, extra pillows to sit up in bed, a wheel chair so the patient can enjoy the garden, or a spouted cup so the patient can drink lying down.

It may be that the patient mowed the lawn, or drove the car for shopping. They may have massaged your stiff shoulder, or cut your toe nails. They may be the only family member who knows how the computer works.

We may be able to help - if we cannot we almost certainly know someone who can. If you have a problem - please talk to your carer.
